Dear Jyoti,
I had a look at your input file and I see that you define
your regions by placing 48 RPP bodies adjacent to each other:
I believe that this is creating problems during tracking due to the
numerical precision of the calculation and generates your error.

In essence, if you have two RPPs with a coincident face, when
a particle is on that face (with a given numerical precision)
to which body will it belong? This is the problem...

You will see many errors that will look like this one:

---
Geofar: Particle in region    19 (cell #    0) in position
1.000000000E-01 -3.247844229E+01  1.000756559E+02

Try again to establish the current region  moving the particle of a
1.420397324E-08 long step
We succeeded in saving the particle:  current region is n.    19 (cell
#    0)
---
beacuse the particle cannot be assigned by FLUKA to any region.
To avoid it, as recommended in the FLUKA manual and in the
slides shown at the courses, it is much better to define a
bigger volume and to segment it with infinite planes: the
various regions can be defined with the available boolean
operators. For an example, you can inspect the default FLUKA
exercise in the installation directory ("example.inp"), in which
a cylindrical target is divided in two regions by an infinite
plane. See also the manual, chapter 8.
Hope this helps and kind regards,
Andrea
