FLUKA is suitable for the purposes you are interested in.
Simulation of medical linacs has been extensively performed with FLUKA main
code.
I have worked a lot with FLUKA studying medical physics issues among them
medical linear accelerators. Moreover, excellent agreement is found between
FLUKA and other MC main codes, like MCNP, PENELOPE, GEANT, etc. There are a
lot of papers on the issue.
