I am not sure to have fully understood, but I'll try to give some answers:
The direction of the beam has to be set in the BEAMPOS card. If I
understand, the beam makes a 5 degrees angle with respect to the z azis.
In which plane? x -z or y-z?
I suppose here x:
BEAMPOS 10. 0.0 19. 0.0872 NEGATIVE
With this, all your particles will be directed at 5 degrees from the -z
direction.

The BEAMAXES card is used to define beam width and divergence in a
coordinate system that is not the standard FLUKA one. I mean, when your
beam widths are not in the x and y directions.

In your message, you say that you need a rectangle along y and z. So, you
need a new reference system in which the new x' axis is along the standard
z axis, y can stay the same, and right-hand rule means that z' is along
-x.
Beamaxes accepts the cosines of the new x' and z' axes wrt the old ones:
BEAMAXES 0. 0. 1. -1. 0. 0.
BEAM -.006 38. 1. 1.HEAVYION


(38 cm along the new x, old z , 1 cm along y)
(cards not aligned..)

Hello Dear FLUKA Experts,

I wanted to simulate a heavy ion beam along the negative z axis which
forms a rectangle along y and z direction 1 x 38 cm2(X axis is the width
of target)_. The beam directed in a direction to the"geometry Z" axis,
at 5 degrees along the negative z axis.

I ask you to check or help, because I think I did it wrong
