Dear Ye,
to create a RTplan you can use the SPOTBEAM, SPOTPOS, SPOTDIR cards
to define multple beam spots.

A detailed description of those cards can be found in the FLUKA manual,
sections 7.68, 7.69 and 7.70.

If many cards are needed (as is often the case), I suggest you to
create an external routine (e.g. in C, C++, python, etc) that generates a
text file of the form of the FLUKA input file, with the SPOTBEAM, SPOTPOS,
SPOTDIR cards and associated parameters.

If you also need to import DICOM images, please take a look here
(slides 18-20):

If you need further support, please let us know.

Best regards
