Dear Shyam,
I assume the file was produced by FLUKA post-processing programs.
That format is then ready for plotting in Flair using gnuplot.
column 1: low edge of the bin (along x-,y- or z-axis depending on which projection you selected)
column 2: up edge of the bin
column 3: DPA (or, more in general, the quantity scored)
column 4: error in percent
I recommend you to study the lectures from FLUKA course, in this case the one on the scoring<

it is reported the meaning of the columns in page 27.
