I’m surprised that you were able to build a FLUKA executable with the source routine you attached, since it does not compile.
Please find enclosed a modified version that I tested with your input.

(from page 25).
In the source.f routine, you have to set the energy through the variable TKEFLK.
Then set PMOFLK accordingly using the formula that is usually commented in the source.f routine available from the FLUKA distribution.
