This way to interpret this output is straightforward; The data is given in (x,y,z) of whatever you are scoring (particle 1 it appears) and spaced according to--in your case--1 cm in the x- and y-directions, and 0.5 cm (fixed) in the z-direction.  You should read the data from left to right in groups of three; The "format" is simply the FORTRAN format the data is written in.
