Thanks for your replies.
I ran the simulation with 50 cores to improve the statistics. I set the 1keV as the threshold foe almost all the area (Target and void).
I see zero erro and Zero dose Equivalent on the other side of the target (opposite side of source) ?!? If switching to single scattering (MULSOPT card) causes this Zeros? The plots and inp files are attached

in EMFCUT you have to specify the regions where the threshold applies,
i.e. TARGET as WHAT(4) in your case
If you look into the output file (NewDesignV5-1001.out), you will see
that your 1 keV threshold has not been taken due to the wrong command
