I forward the answer to the fluka discuss list since that is the proper
forum for these questions.

The message you see is pointing to a geometry problem, that is during
magnetic field tracking the geometry returns an error message because
the region the particle believes to be in is not the correct one.

One possible (obvious) reason could be an error in the geometry, possibly
just a rounding one, even though it is somewhat unusual that it cannot be
overcome. Obviously a macroscopic error in the geometry (eg an "hole" in
it or a macroscopic double defined region of space) would ingenerate
similar messages forever.

In some very pathological situations (unlikely) it could be caused by
too coarse tracking parameters in the magnetic field (MGNFIELD card and/or
STEPSIZE card), but in that case the recovery procedure should have
succeeded.
